The Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual's case.
We must have your signature to proceed with this type of request.
Help with a Federal Agency
Examples of the federal agencies we regularly interact with include the Internal Revenue Service (IRS), the Department of Veterans Affairs, the Social Security Administration, U.S. Citizenship and Immigration Service, and the Department of State- U.S. Passport Agency.
For our office to be able to help you with your case, and to comply with the Privacy Act of 1974 (Public Law 93-579), constituents must fill out a Privacy Release Form (PRF). This document grants us permission to discuss your case with the appropriate federal agency. While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your issue.
Note: Constituents must reach out through the Congresswoman’s webpage, the office helpline, or in person at one of the district offices to receive assistance.
