For us to proceed with your case, we need you to complete the Digital Release Form that you can submit this form electronically. Please return the signed form at your earliest convenience as we cannot engage with the Department of  Veterans Affairs on your behalf until it is completed.

You may provide supporting documentation by uploading and attaching them when submitting your inquiry. Make sure to include the following;

• DD214
• VA Case Number (if applicable)

Once we have received the documentation you will receive an email from our office confirming the submission of your request.
