Passport Help
In order for us to engage with the Department of State to inquire about your U.S. Passport. We need you to complete the Digital Release Form electronically. Please complete and return the signed release form at your earliest convenience as we cannot engage with any federal agency until we have this written consent.

You must also provide any supporting documents pertinent to your case such as:
- A travel itinerary.
- Proof of emergency situation.
- Any receipts you have received while communicating with the agency.
Once we have received the form and supporting documentation we will engage with the DOS on your behalf.
