For us to proceed with your case, we need you to complete the Digital Release Form that you can submit this form electronically. Please return the signed form at your earliest convenience as we cannot engage with the U.S. Social Security Administration (SSA) on your behalf until it is completed.

You may provide supporting documentation by uploading any supporting documents pertinent to your case, especially any communication you have received from the agency. Once we have received the documentation you will receive an email from our office confirming the submission of your request.
