Immigration Help

For us to proceed with your case, we need you to download, print, and hand-sign the Digital Privacy Release Form that is located at the bottom of the page. Please submit the following documentation requested by our office to efficiently process your request. We understand that some documentation might not be applicable or available to you, however, keep in mind that not including this in our initial inquiry might delay your request with the U.S. Citizenship and Immigration Services (USCIS). 

  • Privacy Release Form signed (required)
  • The latest USCIS Receipt Letter (required)

For expedited request attached include the following;

  • The affidavit letter signed by the applicant requesting the Expedited request to USCIS 
  • Any other supporting documents that may be relevant to this request. If you are unsure about what qualifies, please carefully review the How to Expedite Request USCIS guidelines to ensure you meet their criteria. (Please attach: do not forward information)

Please return the documents mentioned above at your earliest convenience to il03constituentservices@mail.house.gov as we cannot engage with USCIS on your behalf until it is completed. If you do not have access to print and scan, please call to schedule a visit to one of our offices so we can assist you directly.

Once we have received the documentation you will receive an email from our office confirming the submission of your request.

Note: USCIS would deny processing any inquiry that has a digital signature and/ or any third-party requesters.
